Connect to the clinic on Libre
Sharing your FreeStyle Libre data lets Dr Chan review your glucose readings before and between appointments. It takes about a minute, and the practice ID is Health111. Using a Dexcom instead? See the Dexcom Clarity connection guide.
How to share your Libre data
- Log in to your account at www.libreview.com.
- Click the three lines (menu) in the top-right corner.
- Select Account Settings.
- Select My Practices.
- Enter Health111 as the Practice ID, then click Add.
You can remove access at any time from the same My Practices screen. Your data remains private and is only visible to the clinic.
